Montecito and Montvale behaves slightly differently than previous
Itanium processors, resulting in the MCA due to a failed PIO read
to sometimes surfacing outside the nofault code. This code is
based on discussions with Intel CPU architects and verified at
customer sites.

The Altix shub2 BTE error detail bits are in a different location
than on shub1. The current code does not take this into account
resulting in all shub2 BTE failures mapping to "unknown".
This patch reads the error detail bits from the proper location,
so the correct BTE failure reason is returned for both shub1
and shub2.

When initializing pci_controller->node to point to the closest node we need
to take into consideration that a PIC PCI Bridge ASIC can be connected to a
headless/memless node just like the TIOCP and TIOCE Bridge ASICs

Currently, XPC's heartbeat timer function runs on whatever CPU modprobe/insmod
ran on when XPC was started. To avoid the heartbeat from being delayed for
long periods the timer function must run on CPU 0.
N.B. Altix doesn't currently allow cpu0 to be taken offline, so this is
safe for now. This code must be revised when offline of cpu0 is enabled.

When the CPE handler encounters too many CPEs (such as a solid single
bit memory error), it sets up a polling timer and disables the CPE
interrupt (to avoid excessive overhead logging the stream of single
bit errors). disable_irq_nosync() calls chip->disable() to provide
a chipset specifiec interface for disabling the interrupt. This patch
adds the Altix specific support to disable and re-enable the CPE interrupt.

This changes the uevent buffer functions to use a struct instead of a
long list of parameters. It does no longer require the caller to do the
proper buffer termination and size accounting, which is currently wrong
in some places. It fixes a known bug where parts of the uevent
environment are overwritten because of wrong index calculations.
Many thanks to Mathieu Desnoyers for finding bugs and improving the
error handling.

Add additional support for CPU disable on SN platforms.
Correctly setup the smp_affinity mask for I/O error IRQs.
Restrict the use of the feature to Altix 4000 and 450 systems
running with a CPU disable capable PROM, and do not allow disabling
of CPU 0.

If the interrupt has been disabled, don't call the force_interrupt provider.
Doing so can result in an infinite runaway interrupt loop.

If the sn2_rtc clock is present then it is a must have since sn2_rtc
provides a synchronized time source on Altix systems. So elevate
the priority to 450. Otherwise the ITC would take precendence. Altix
systems currently do not boot because the ITC clocksource is broken. It
seems to assume that ITCs are synchronized and as a result nanosleep
hangs (may be fixed in a different patch).
While we are at it: Remove the sn2_mc definition. The sn2_rtc has a fixed
address. No point in reading the address from memory. Removing it avoids
touching one cacheline.

On SN systems, when setting the IORESOURCE_ROM_BIOS_COPY resource flag,
the resource length should be set to the actual size of the ROM image
so that a call to pci_map_rom() returns the correct size.

This one changes the SN2 specific PCI drivers to use ioremap() for
obtaining the real address to access for the PCI registers instead of
manually calculating them with __IA64_UNCACHED_OFFSET.
The patch should have no real change when running on a normal Linux
kernel, but when running as a paravirtualized it is needed.

This patch moves the die notifier handling to common code. Previous
various architectures had exactly the same code for it. Note that the new
code is compiled unconditionally, this should be understood as an appel to
the other architecture maintainer to implement support for it aswell (aka
sprinkling a notify_die or two in the proper place)
arm had a notifiy_die that did something totally different, I renamed it to
arm_notify_die as part of the patch and made it static to the file it's
declared and used at. avr32 used to pass slightly less information through
this interface and I brought it into line with the other architectures.

When redirecting a device interrupt on SN, not all links
between platform specific structures are being updated.
This can result in a system crash if an interrupt
redirection is followed by an unplug of that device.
The complete fix also requires a prom update. Though,
this patch is backward compatable and not dependent on
the prom patch.

set_irq_msi() currently connects an irq_desc to an msi_desc. The archs call
it at some point in their setup routine, and then the generic code sets up the
reverse mapping from the msi_desc back to the irq.
set_irq_msi() should do both connections, making it the one and only call
required to connect an irq with it's MSI desc and vice versa.
The arch code MUST call set_irq_msi(), and it must do so only once it's sure
it's not going to fail the irq allocation.
Given that there's no need for the arch to return the irq anymore, the return
value from the arch setup routine just becomes 0 for success and anything else
for failure.

I noticed that many source files include <linux/pci.h> while they do
not appear to need it. Here is an attempt to clean it all up.
In order to find all possibly affected files, I searched for all
files including <linux/pci.h> but without any other occurence of "pci"
or "PCI". I removed the include statement from all of these, then I
compiled an allmodconfig kernel on both i386 and x86_64 and fixed the
false positives manually.
My tests covered 66% of the affected files, so there could be false
positives remaining. Untested files are:

I would welcome test reports for these files. I am fine with removing
the untested files from the patch if the general opinion is that these
changes aren't safe. The tested part would still be nice to have.

So that it is also an offset from skb->head, reduces its size from 8 to 4 bytes
on 64bit architectures, allowing us to combine the 4 bytes hole left by the
layer headers conversion, reducing struct sk_buff size to 256 bytes, i.e. 4
64byte cachelines, and since the sk_buff slab cache is SLAB_HWCACHE_ALIGN...
:-)
Many calculations that previously required that skb->{transport,network,
mac}_header be first converted to a pointer now can be done directly, being
meaningful as offsets or pointers.

In sn_io_slot_fixup(), the parent is re-set from the bus to
io(port|mem)_resource because the address is changed in a way that it's not
child of the bus any more.
However, only the root is set but not the parent/child/sibling relationship in
the resource tree which causes 'cat /proc/iomem' to stop after this memory
area. Depding on the poition in the tree the iomem may be nearly completely
empty.

When booting an SN system without specifing a console
(i.e., no "console=" on boot line), the system will hang during
boot at the point where /sbin/init is run.
The problem is that vga_console_iobase is not converted to a
virtual address before storing in io_space[0].mmio_base.
The conversion was happening in sn_scan_pcdp(), but not in
setup_vga_console().

To provide compatibilty with SN kernels that do and do not
have ACPI IO support, the SN PROM must build different
versions of some ACPI tables based on which kernel is booting.
As such, the tables may have to change at kernel boot time.
By default, prior to kernel boot, the PROM builds an empty
DSDT (header only) and no SSDTs. If an ACPI capable kernel
boots, the kernel will notify the PROM, at platform setup time,
and the PROM will build full DSDT and SSDT tables.
With the latest changes to acpi_table_init(), the table lengths
are saved, and when our PROM changes them, the changes are not seen,
and the kernel will crash on boot. Because of issues with kexec support,
we are not able to create the tables prior to acpi_table_init().
As a result, we are making a second call to acpi_table_init() to
process the rebuilt DSDT and SSDTs.
