This adds two little devicetree helper functions for determining the
dr_mode (host, peripheral, otg) and phy_type (utmi, ulpi,...) from
the devicetree.

otg.c only had a single function definition
which might make more sense to be placed in
usb-common.c. While doing that, we also delete
otg.c since it's now empty.

all other functions under drivers/usb/ start
with usb_, let's do the same thing.
This patch is in preparation for moving otg_state_string
to usb-common.c and deleting otg.c completely.

During phy interface separation from otg.h, as the enum "usb_otg_state"
was having multiple otg states info and removal of member 'state'
of this enum type from usb_phy struct did not generate any compilation
issues, I removed member state from struct usb_phy.
As this is causing build break in musb code, adding member 'state'
to usb_phy structure.

function prototypes. Move those prototypes to

Used devres API's to associate the phy with a device so that on
driver detach, release function is invoked on the devres data(usb_phy)
and devres data(usb_phy) is released.

Add a linked list for keeping multiple PHY instances with different
types so that we can have separate USB2 and USB3 PHYs on one single
board. _get_phy_ has been changed so that the controller gets
the transceiver by type. _remove_phy_ has been added to let the phy
be removed from the phy list.

_transceiver() in otg.c is replaced with _phy. usb_set_transceiver is
replaced with usb_add_phy to make it similar to other usb standard
function names like usb_add_hcd.

musb can be suspended at the time some other driver wants to do ulpi
transfers using usb_phy_io_* functions, and that can cause data abort,
as it happened with isp1704_charger:

Add pm_runtime to ulpi functions to rectify this. This also adds io_dev
to usb_phy so that pm_runtime_* functions can be used.

This changes the otg functions so that they receive struct
otg instead of struct usb_phy as parameter and
converts all users of these functions to pass the otg member
of their usb_phy.
Includes fixes to IMX code from Sascha Hauer.

Introducing struct otg and collecting otg specific members
to it from struct usb_phy. There are no changes to
struct usb_phy at this stage. This also renames
transceiver specific functions, and offers aliases for the
old otg ones.

Commit 3dacdf11 "usb: factor out state_string() on otg drivers"
broke building musb drivers since there is already another
otg_state_string() function in musb drivers, but with different
prototype. Fix musb drivers to use common otg_state_string(), too.
Also provide a nop for otg_state_string() if CONFIG_USB_OTG_UTILS
is not defined.

Save the last event in the otg_transceiver so that it can used in the
musb driver and gadget driver to configure the musb and enable the
vbus for host mode and OTG mode, if the device is connected during boot.

If the NOP USB OTG transceiver driver is built as a module, the otg.h
header declares external functions, but if they are referenced from the
kernel proper, as, e.g., in the OMAP3 case, where the omap3evm board is
calling the usb_nop_xceiv_register() function, linkage breaks. This patch
fixes this problem.

The notifier will be used to communicate usb events
to other drivers like the charger chip.
This can be used as source of information to kick
usb charger detection as described by the USB
Battery Charging Specification 1.1 and/or to
pass bMaxPower field of selected usb_configuration
to charger chip in order to use that information
as input current on the charging profile
setup.

Add empty functions for usb_nop_xceiv_register()
and usb_nop_xceiv_unregister() in otg.h so that
these functions can be called even when
CONFIG_NOP_USB_XCEIV is not enabled.
It allows to remove ifdef's from board file.

This adds support for OTG transceivers directly connected to the ULPI
interface. In particular, the following details are added
- a struct for low level io functions (read/write)
- a priv field to be used as 'viewport' by low level access functions
- an (*init) and (*shutdown) callbacks, along with static inline helpers
- a (*set_vbus) callback to switch the port power on and off
- a flags field for per-transceiver settings
- some defines for the flags bitmask to configure platform specific
details

The NOP OTG transceiver driver needs to be usable from modules.
Make sure its symbols are always accessible at both compile and
link time, and make sure the device instance is allocated from
the heap so that device lifetime rules are obeyed.

NOP transceiver is used by all the usb transceiver which are mostly
autonomous and doesn't require any programming or which are built
into the usb ip itself.NOP transceiver only allocates the memory
for struct xceiv and calls otg_set_transceiver() so function call
to otg_get_transceiver() will return a valid transceiver.
NOP transceiver device should be registered by calling
usb_nop_xceiv_register() from platform files.

workqueue rather than from an irq handler, allowing msleep()

inclusion protection and standardize what's already there. The
apparent standards:
* macro name of __LINUX_USB_headerfile_H
* inclusion protection placed after leading comment block
* macro name added as a comment on the final #endif
* any obvious trivial whitespace cleanup associated with the above
