Several files refer to an old address for the Free Software Foundation
in the file header comment. Resolve by replacing the address with
the URL <http://www.gnu.org/licenses/> so that we do not have to keep
updating the header comments anytime the address changes.

Move the functions into a separate module, in order
to make those usable from other modules. Also move
the queue register offset macros from rt2800pci.h
into rt2800mmio.h.

This patch support the new chipset rt3290 wifi implementation in rt2x00.
It initailize the related mac, bbp and rf register in startup phase.
And this patch modify the efuse read/write method for the different efuse data offset of rt3290.

There is no point on having them separated across 3 files.
At the same time rename USB_CYC_CFG to its proper name US_CYC_CNT
(as per the datasheet).

As mentioned on the linux-wireless mailing list, the current copyright
statements in the rt2x00 are meaningless, as the rt2x00 project is
not even a formal legal entity. Therefore it is better to replace
the existing copyright statements with copyright statements for the
people that actually wrote the code.
Note: Updated to the best of my knowledge with respect to who
contributed considerable amounts of code.

eFuse EEPROM is used also by USB chips (i.e. RT3070)
so move the needed code from rt2800pci to rt2800lib.

Instead of assuming that all rt3090 devices will have an eFuse EEPROM,
do as the legacy Ralink driver, and detect at run-time whether an
eFuse EEPROM is present.
