Replace the current semi-arbitrary distribution of inline assemblies:
- Inline assemblies used by CIO go into ioasm.h
- Data definitions used by inline assemblies go into cio.h
Beyond cleaning up the current structure this is also required for
use of tracepoints in inline assemblies introduced by a follow-on
patch.

The numeric representation of the xsch instruction was incorrectly
specified, resulting in reserved fields of the instruction opcode
potentially being set to a non-zero value. While this doesn't currently
cause any problem, a future architecture might make use of these fields
so that the current specification could result in an exception or
unwanted side-effects.
Fix this by using the xsch instruction code for which support in
binutils was added in 2003.

Do not abort path verification when waiting for an interrupt timed out.
Use path_noirq_mask to keep track of the paths used for this (also
maintain a path_notoper_mask for debugging purposes). If the timeout
happend to be during an operation where we query or alter the state of
path groups set the pgid_unknown flag.
With this change we allow usage of devices which have such ill-behaved
paths (if at least one path is operational).

If a driver wants to do command mode IO while CIO is doing
online path verification we ignore this request and provide
a fake irb when we are done and the driver can do IO again.
For transport mode IO we have no such mechanism, giving the
driver no other chance then to retry the action until we are
done. This is not very reliable.
Provide a fake irb for transport mode IO as well.

Current IRQ statistics support does not show detail counts for I/O
interrupts which are processed internally only. The result is a
summation count which is way off such as this one:

Move the subchannels ccw device pointer from drv_data to
the private data for I/O subchannels, since it is not the
only drv_data for I/O subchannels. The next step will be
to make io_subchannel_private the new drv_data for I/O
subchannels.

Change the ccwrequest infrastructure to use more than one channel
path per start I/O. A flag "singlepath" is added to struct
ccw_request - if set, the old behavior is used. This flag is set
for all exploiters of the ccwrequest infrastructure - so there
is no functional change through this patch.

Channel path vary is currently broken: channel paths which are varied
offline are still used by Linux. The reason for this is that:
* the path mask indicating which paths of an I/O device can be used
is reset by each internal I/O request
* the logic that checks if a path group is already in its designated
target state incorrectly interprets the result "is correctly set"
as "is correctly set and available"
Fix this by resetting the path mask only for internal I/O requests
which affect the path mask and by correcting the pgid check logic.

Improve error recovery for internal I/Os by repeating each I/O
256 times per path to cope with long-running non-permanent error
conditions. Also retry each path twice to cope with link flapping,
i.e. single paths becoming unavailable in the order in which they
are tried.

The function ccw_device_unregister has to ensure to remove
all references obtained by device_add and device_initialize.
Unfortunately it gets called for devices which are
1) uninitialized, 2) initialized but unregistered, and
3) registered devices. To distinguish 1) and 2) this patch
introduces a new flag "initialized", which is 1 as long as we
hold the initial device reference.

We used to maintain a "registered" flag in our ccw_device_private
structure. This patch removes the "registered" flag and converts
all users of it to device_is_registered which has the exact same
meaning.
Note: The usage the atomic operation test_and_clear_bit is replaced
by the non-atomic if (device_is_registered()) device_del(). This
will not do harm, since we serialize calls to ccw_device_unregister
with a single-threaded workqueue.

Set-pgid operations fail for some device types under z/VM for which
the hypervisor has already set the pgid. Also reserved devices or
changed pgids are not correctly recognized. Fix these problems by
using a combination of sense-pgid and set-pgid and by also accepting
pre-defined pgid settings.

Split setting (driver wants feature enabled) and status (feature
setup was successful) for PGID related ccw device features so that
setup errors can be detected. Previously, incorrectly handled setup
errors could in rare cases lead to erratic I/O behavior and
permanently unusuable devices.

After changing all internal I/O functions to use the newly introduced
ccw request infrastructure, retries are handled automatically after a
clear operation. Therefore remove the internal retry flag and
associated code.

Introduce a central mechanism for performing delayed ccw device work
to ensure that different types of work do not overwrite each other.
Prioritization ensures that the most important work is always
performed while less important tasks are either obsoleted or repeated
later.

This patch adds a driver for subchannels of type chsc.
A device /dev/chsc is created which may be used to issue ioctls to:
- obtain information about the machine's I/O configuration
- dynamically change the machine's I/O configuration via
asynchronous chsc commands

Introduce a private pointer in struct subchannel to store
per-subchannel type data (cannot use dev->priv since this
is already used for something else).
Create a new header io_sch.h for I/O subchannel specific structures
and instructions.
