When the OS/2 driver hits a disk write error, it writes the sector to
another location and adds the sector mapping to the hotfix map.
This patch makes the hpfs driver understand the hotfix map and remap
accesses accoring to it.

This patch implements prefetch to improve performance. It helps mostly
when scanning the bitmaps to calculate free space.

The test if bitmap access is out of bound could errorneously pass if the
device size is divisible by 16384 sectors and we are asking for one bitmap
after the end.
Check for invalid size in the superblock. Invalid size could cause integer
overflows in the rest of the code.

Initial git repository build. I'm not bothering with the full history,
even though we have it. We can create a separate "historical" git
archive of that later if we want to, and in the meantime it's about
3.2GB when imported into git - space that would just make the early
git days unnecessarily complicated, when we don't have a lot of good
infrastructure for it.
Let it rip!
