Moves the Mellanox driver into drivers/net/ethernet/mellanox/ and
make the necessary Kconfig and Makefile changes.

For older FW versions, fixing the usage of per port Mac table.
For each port we must define the base QP number, which is passed
to the HW.
Setting the correct value in SET_PORT FW command to enable the steering.

Query another dword containing up to 32 extended device capabilities
and merge it into struct mlx4_caps.flags. Update the code that
handles the current extended device capabilities (e.g UDP RSS, WoL,
vep steering, etc) to use the extended device cap flags field instead
of a field per extended capability. Initial patch done by Eli Cohen

The mlx4_en module now uses the new steering mechanism.
The RX packets are now steered through the MCG table instead
of Mac table for unicast, and default entry for multicast.
The feature is enabled through INIT_HCA

Change multicast promiscuous mode to pass packets through the multicast group distribution table
before sending packets that miss to the default multicast QP.

The selftest includes 5 features:
1. Interrupt test: Executing commands and receiving command completion
on all our interrupt vectors.
2. Link test: Verifying we are connected to valid link partner.
3. Speed test: Check that we negotiated link speed correctly.
4. Registers test: Activate HW health check command.
5. Loopback test: Send a packet on loopback interface and catch it on RX side.

The per ring counters are implemented in SW. Now moving to have the total
counters as the sum of all rings. This way the numbers will always be consistent
and we no longer depend on HW buffer size limitations for those counters
that can be insufficient in some cases.

The Mellanox ConnectX can operate as an InfiniBand adapter, as an
Ethernet NIC, or as a Fibre Channel (FC) HBA. The kernel has a
low-level driver, mlx4_core, which handles multiplexing access to the
device, and there is also already an InfiniBad driver, mlx4_ib.
This patch adds a new driver, mlx4_en, which implements a standard
Ethernet NIC driver.
